Next to him are two soldiers, the physician Philip, and a child who, with a silent gesture, invites discretion. Alexander's nearly naked body highlights his fragility, while the juxtaposition with the child and the dark setting add a sense of delicacy and mystery to the scene.
The painting, inspired by the school of Lanfranco, uses chiaroscuro to create a strong contrast between light and shadow, heightening the drama of the scene. The scene displays a strong sense of spatiality, with a dramatic rendering of Alexander the Great's face and body, reminiscent of the Roman Baroque style.
